I'll answer those questions with the hypothetical idea that Castle gets would have gotten shot in the same way Kate did.

Would Beckett still be with Josh? No. She would have broken up with him.

Would Castle have told Becket he loved her before he passed out? No, like Kate he wouldn't have had the strength to say anything.

Would Beckett have told Castle that she loved him? No, she would have been desperate and would have begged him to stay alive but she wouldn't have told him that she loved him.

Would she tell him that she heard his I Love You or would she lie to him? –> 2nd question.

Would Castle lie to Beckett like she has to him and say he did not remember saying he loved her?
–> 2nd/3rd question.

Would Beckett start changing like she is now doing? No, she would have pulled away from Castle because she would have felt extremly guilty.

Would she still be investigating her mothers murder case or would she change the way she would go about it? She would be even more obsessed about it and probably would have gotten herself killed during the summer.

Would either or both of them be seeing a Pyschologist/or Psychiatrist? Maybe Castle but I don't Kate would have crawled inside the rabbit hole even more.

I'd guess that Beckett would NOT tell Castle she loves him, only to have him nearly die, but recover in the end.  This would leave her terrified of losing him again without her telling him she loves him, so she go down a similar track to that which she is now pursuing: working to put her baggage and previous issues behind her so she can work towards a relationship with him.  As a result, she would have dumped Josh, she would be seeing a psychologist, and she would start changing.
